New Client Check List

In your attorney/client confidential meeting with our firm, the attorney will provide an honest assessment of the merits of your personal injury case.  Evidence is key to properly evaluating your personal injury case. Below is  a list of evidence that will assist in the intial evaluation process with our firm.

PLEASE PRINT THIS LIST AND BRING THE FOLLOWING INFORMATION TO YOUR FIRST MEETING

◻ Names and contact information of the responsible parties
◻ Any witness contact information
◻ Any police or incident reports
◻ Your automobile insurance information if your case involves an auto accident.
    Specifically we need the Declaration Page from your insurance policy.
◻ An estimate of repair to your vehicle
◻ Any photographs of damage to your vehicle
◻ Any photographs of the scene of the accident
◻ The exact location of the incident
◻ Photographs of injuries that occurred as a result of the accident
◻ The names and addresses of any medical providers that you have seen since the accident
